Evening Residential Nurse

Guild South

Full time | South St. Paul, MN

This position is responsible for providing nursing services in a residential setting for individuals who have a serious mental illness and/or a substance use disorder. This position assists with the admission process including screening referrals, completing assessments, and helping the team determine immediate and ongoing recipient needs. This position provides a full range of nursing services and oversees the health management of all individuals served (15 total.)

Care Coordinator

Care Coordination

Full Time | St. Paul, MN

The Care Coordinator position is responsible to provide advocacy for program members while helping them navigate complex social service and health care delivery systems. The Care Coordinator will assess member needs and develop a comprehensive plan of care. This position works alongside other care team providers and supports to ensure effective and well-coordinated care for all enrolled members. Care Coordination services are provided under a delegation agreement with a health insurance plan and this position is responsible for completing all health plan trainings, documentation, and connecting members to health promotion programming and resources.

Director of Integrated Services

Guild is seeking an innovative and passionate Director to lead our Housing, Homelessness, and Employment Services.

Reporting to the Chief Clinical Officer, this Director is responsible for the overall management and delivery of the following service areas: Supportive Housing, Housing Case Management, Homeless Outreach, and Employment Services. This position will lead teams responsible for ensuring that services are effective, efficient, compliant, sustainable, and bring about positive client outcomes in optimal health and quality of life. This position works as part of Guild’s Executive Team to ensure quality services align with Guild’s mission and strategic vision.

Cook/Household Coordinator

Guild Crisis and Recovery Center, Scott County

Full Time | Savage, MN

This position is responsible for coordinating and managing all aspects of meal planning and preparation, shopping and supplies management, and staff and resident kitchen maintenance for sixteen IRTS and Crisis Clients.

Housing Case Manager

Dakota Rapid Rehousing

Full Time | St. Paul, MN

The Dakota Rapid Rehousing Team serves individuals and families that have been assessed in Dakota County and are currently on the Coordinated Entry waitlist. The program is designed to assist individuals to pay for housing for a short period of time that enables them to move quickly out of homelessness and stabilize them in permanent housing. Staff provide housing search assistance, connection to mainstream services/benefits, and provide housing case management.

Mental Health Professional (Clinical Trainee)

Community Access

Full Time | St. Paul, MN

This position is a fast-paced and dynamic role that supports Guild’s Community Access program. Community Access acts as an initial point of contact for individuals who are interested in receiving mental health, housing, and employment services or learning about community resources and how to navigate the health care system. In this position, the Clinical Trainee/Mental Health Professional will complete diagnostic assessments and make level of care determinations, provide housing consultation evaluations, and engage in solution-focused conversations with callers and community members. The Clinical Trainee/Mental Health Professional also provides therapeutic intervention, skill development, and coordination of care for families and ongoing clients through a private pay model of service. The goal of this position is to increase access and knowledge of services and resources and promote the independence and resiliency of individuals.

Facilities Maintenance

Full Time | St. Paul, MN

The maintenance worker is a vital contributor to the facilities team under the direction of the facilities manager. The maintenance worker will support multiple locations. Facility services utilize work ticket systems, email, and one on one communication to coordinate with affected administrative, clerical, and program associates to reduce disruption and expedite resolution of facility enhancements and concerns.

On a day-to-day basis, the facilities team is responsive needs to both routine maintenance and/or unique improvements, projects, and repairs. This includes a variety of responsibilities which not only includes maintaining a clean and safe environment both within and outside of properties, but also HVAC & electrical systems, small appliances, company vehicles, lighting, furnishings and finishes, supplies, fire, and life safety, and security systems.
