Guild’s Community Access team provides resources that can prevent mental health problems from escalating and spiraling into a crisis that can lead to homelessness or hospitalization. Community Access will help you navigate through systems to understand what resources are available so you can maintain stability, safety, and success.

Community Access begins with an initial phone screening or this online inquiry form. From there, our team will help you identify your needs and complete a Diagnostic Assessment, enabling us to refer you to ongoing services including ACT, TCM, and BHH.  

Community Access Services help:

  • Provide assessment and planning.
  • Give therapeutic intervention.
  • Access the necessary resources to meet your goals
  • Provide the necessary referrals.

Services to meet your needs

Services are also available on a Private Pay basis to provide support, consultation, and education for family members or concerned others of individuals who are experiencing mental illness.
