Youth Assertive Community Treatment (ACT) Services help individuals ages 14-21 who are living with mental illness find success in education, employment, and community living. Designed to promote optimal health and success, our Youth ACT Services are a comprehensive and community-based approach to serving those with diagnosed mental illness(es) and co-occurring substance use disorders or other medical conditions.

By walking alongside individuals with schizophrenia, bipolar disorder, or another psychiatric illness, Guild works to promote recovery, resilience, and wellness. We want to help you find success–whatever that means to you. We know that mental health issues can interfere with everyday life, which is why we help you build support networks so you can successfully manage everyday challenges.

Youth ACT Services:

  • Help stabilize symptoms and provide medication management and education. 
  • Identify available resources to meet your immediate needs.
  • Help experience success in work, school, or volunteer opportunities.
  • Develop your own personal support system.
  • Help stabilize your housing.
  • Provide 24/7 support for crisis intervention. 
  • Offer family support and education.  
  • Provide individual and group therapy.

Services to meet your needs

ACT is an evidence-based practice that is shown to decrease the need for hospitalization and increase the likelihood of maintaining a stable living situation. Our services are driven by clients, community-focused, and team-based. Unable to commute to our locations? Don’t worry. We’ll meet with you at times and locations that are convenient for you at least three times per week as needed.

Our Youth ACT team consists of Licensed Mental Health Professionals, a Psychiatrist, Peer Recovery Specialist, Nurse, Licensed Alcohol and Drug Counselor, and an Education Specialist. By covering various aspects of wellness, we help youth make lasting connections, find creative solutions to challenges, and set purposeful goals.

Eligibility

To qualify for Guild’s Youth ACT Services, you must:

  • Live in the Twin Cities Metro area.
  • Be 14-21 years old.
  • Have a diagnosed mental illness with symptoms that interfere with everyday life and a need for high-intensity services.
  • Have a health insurance plan that covers Youth ACT Services.
