Care Coordination helps you access health services and resources. We partner with you to develop a holistic health plan–physical, medical, dental–to help manage your long-term health. By creating a personalized plan, you can meet your goals and spend more time enjoying life.

A holistic approach to care

Care Coordination will help you identify your health and wellness goals and develop a comprehensive plan of care. By working with your health plan and primary care provider, we can help you understand your mental health and how to manage it. Everyone involved in your care will be on the same page, ensuring that you experience service stability and consistency.

Care Coordination Services help:

  • Identify, understand, and manage your health needs.
  • Access wellness and health support including regular preventative medical care, dental care, and medications.
  • Find and provide social services and support.
  • Coordinate transportation to healthcare appointments.

Services to meet your needs

Guild’s Care Coordination team includes Mental Health Practitioners that are supervised by a Mental Health Professional that coordinate with community providers, social services, and other support systems. Our services are mobile and provided when and where you need them.

Care Coordination is included when you are enrolled in Special Needs Basic Care through Medicaid. There is no additional charge to you.

Eligibility

To qualify for Guild’s Care Coordination, you must:

  • Receive Medical Assistance.
  • Have a recognized disability.
  • Be referred to Guild by your health plan.
