Our Projects for Assistance in Transition from Homelessness program (PATH) exemplifies our commitment to reaching those in need by offering vital services to individuals who are homeless or on the brink of homelessness. Through PATH, we bring help directly to the streets, ensuring that no one is left behind.

On the Frontlines: The Power of Outreach

PATH’s outreach services are the heart of this initiative. Our outreach workers use a proactive approach to build trust and rapport by meeting people experiencing homelessness wherever they are and addressing their immediate needs.

“Our outreach team is committed to finding and engaging with individuals who might not otherwise have access to essential services,” says Diana Harris, Director of Clinical Services — Coordinated Health & Housing at Guild. “It’s about building trust and making sure they know we’re here to help.”

Tailored Case Management: Guiding the Way to Stability

After establishing initial contact, PATH provides ongoing, personalized case management. Our case managers work closely with clients to navigate the complex web of available services, offering referrals to mental health care, substance use treatment, and housing support. By tailoring our approach to each individual’s circumstances, we ensure that our clients receive the most effective support possible.

Diana notes, “Every person’s journey is unique, and our case managers are trained to address their specific needs and challenges, providing a personalized pathway to stability.”

Holistic and Client-Centered

The PATH program is rooted in a holistic, client-centered philosophy. We recognize that each person’s journey is unique, and this method helps us foster our clients’ resilience.

“Our philosophy is to empower our clients by focusing on their strengths and goals,” Diana says. “We want them to be active participants in their own recovery.”

Overcoming Barriers to Housing

Securing stable housing can be an uphill battle for individuals with serious mental illnesses. PATH assists clients in overcoming these obstacles by helping with applications for benefits such as Social Security and state financial assistance. Our team also collaborates with housing providers to ensure clients find and keep safe, permanent homes.

“Helping our clients navigate the benefits system is crucial,” Diana explains. “It’s often the financial support component that makes the difference between homelessness and a stable home.”

Expanding Our Reach: Serving More Communities

Since its launch in 2020, the PATH program has significantly expanded our ability to serve those in need. With a dedicated outreach team now active in Anoka and other counties, we are better equipped to engage with homeless populations across a broader area. 

“Expanding our outreach efforts means we can serve more individuals and provide the comprehensive support they need to transition out of homelessness,” Diana notes.

The impact of the PATH program is best seen in the transformations of the individuals we serve. Many have moved from the streets into stable housing, received necessary mental health care, and found new hope in their lives.
