What is a Diagnostic Assessment?
A Diagnostic Assessment is often the first step in getting mental health support. During this appointment, a licensed mental health professional takes time to learn about your personal history, current concerns, symptoms, and life circumstances.

What to Expect
- Call: Contact our Community Access team. We’ll discuss your needs and confirm your payment option.
- Scheduling: We’ll respond within days to get you scheduled.
- Assessment: You’ll meet with a clinician to discuss your history and current needs.
- The Path Forward: Within days of your appointment, you’ll receive a completed assessment to use for treatment planning or service eligibility.

Payment & Coverage
We believe in price transparency and accessibility.
- Medical Assistance (MA): We accept Medical Assistance (MA) and all Medical Assistance-based programs (including Medica, Blue Cross Blue Shield, etc.).
- Self Pay: For those not qualifying for MA or preferring to pay out-of-pocket, we offer a flat rate. Please contact Community Access to talk more about your situation and pricing.
Frequently Asked Questions
Do I need a referral?
No. Individuals and families can self-refer by calling our Community Access line. We also welcome direct referrals from case workers, hospital discharge planners, and nonprofit partners.
Is the assessment valid for Case Management eligibility?
Yes. Our Diagnostic Assessments meet all state requirements for determining eligibility for mental health case management and other essential services.
Can I do this from home?
Absolutely. We offer secure, virtual assessments for anyone residing in the state of Minnesota.
What if I’m new to the mental health system?
