Integrated Support for Youth
Navigating Mental Health and
the Transition to Adulthood

Guild provides specialized, community-based services for teens and young adults with serious mental illness (SPMI). Our integrated approach combines intensive mental health treatment (Youth ACT/IRMHS) with employment and life skills support, empowering youth to achieve success in education, work, housing, and social development as they transition to independence.

Youth ACT/IRMHS

Youth Assertive Community Treatment (ACT) & Intensive Rehabilitative Mental Health Services (IRMHS) provides comprehensive, community-based support for youth ages 14–20 living with serious and persistent mental illness (SPMI). Our multidisciplinary team—including a prescriber, nurse, clinicians, and peer support—meets youth where they are to deliver case management, individualized therapy, 24/7 crisis support, and family education. We help youth succeed in school, work, and community life.

Youth Employment & Education

Our team works with young adults (ages 16–24 in Dakota County with a mental health concern) to help them find, get, and keep competitive jobs, and thrive in education. We use the evidence-based IPS model to make a plan, connect individuals with job opportunities, and coordinate support with their school or counselor to reach their career goals.
